Something you could also mention, which I know from rabidsamfan: those undershirts were, I believe, often partially visible. The collar, cuffs and shirtfronts--everything that you could see with a waistcoat and jacket on--were separate pieces, but if a man took off his jacket, those shirtsleeves would be the closest layer to his skin. I THINK. As previously stated, my ideas on all of this are tenuous at best, but that'd be why collars and cuffs were their own thing, to the best of my understanding.
